Community Connections Positions

Life Skills Coach (LSC) - Residential
The LSC provide services to all residents living within the West Haven Community House Residential Program in the areas of daily living activities and independent life skills. The coach implements behavior programs; facilitates and promotes community integration, and is expected to maintain a clean, safe and healthy environment for the residents.

Requirements include:

  • Must be 21 years of age or older
  • Minimum of an Associate’s degree and 1 year experience or High School Diploma and two years of related experience with adults with intellectual disabilities
  • DDS Medication Administration Certification, or the ability to obtain and maintain within six months of hire
  • Certification in CPR required prior to hire.
  • A valid CT driver’s license for for three or more years, and the ability to obtain and maintain approval from vehicle leasing vendor

Available Shifts:
Mon - Fri: 3:00pm - 11:00pm
Mon - Fri: 6:00am - 9:00am
Sat/Sun: 3:00pm - 11:00pm
Sat/Sun: 9:00am - 3:00pm
Sat/Sun: 7:00am - 3:00pm

Salary: $14.75 per hour, DOE


Life Skills Coach (LSC) Ė DSO
The LSC must be creative, caring individual to provide services for adults with intellectual disabilities and behavioral challenges. Coaches are expected to pick up and drop off clients when scheduled, and drive clients on field trips using agency vehicle, and are required to obtain and maintain Public Service License.

Requirements include:

  • Must be 21 years of age or older
  • Minimum high school degree with two years' experience with individuals with intellectual disabilities, or
  • College degree and a minimum of one year experience with individuals with intellectual disabilities
  • A valid CT driver's license for three or more years, and the ability eligible to obtain and maintain a Public Service License and approval from vehicle leasing vendor
  • Proof of personal auto insurance coverage meeting state requirements
  • Eligible to obtain and maintain a public service license and approval from lease vehicle company
  • Ability to drive a 12 passenger van

Hours: Mon-Fri 8:45am - 3:30pm

Starting Salary: $14.75 per hour


Life Skills Coach (LSC) – The Avenues
The LSC provides direct care and support to individuals with intellectual disabilities between the ages of 18 and 21. The LSC is expected to work independently throughout the program day, flexing the daily schedule to meet the needs of the individuals being served. In addition, the coach works as part of a team committed to meeting the needs of clients in the areas of activities of daily living (ADL), socialization, behavior management, personal safety and goal achievement.

Requirements include:

  • Must be 21 years old or older.
  • High school diploma and two years of related experience with young adults with intellectual disabilities
  • A valid CT driver's license for three years or more; eligible to obtain and maintain a Public Service License and approval from vehicle leasing vendor
  • CPR certified

Shift: Mon - Fri: 8:00am – 4:00pm

Salary: $14.75 per hour

 

Assistant Program Manager - Residential
The Assistant Program Manager rovides leadership and support to the Residential Supervisors to meet the program and client needs, and is responsible for ensuring that the residential program complies with all licensing, regulatory and DDS requirements. The Assistant Program Manager works with the Program Manager to ensure that IP goals are met and monitors the health and safety of all residents.

Requirements include:

  • Bachelor’s degree in Human Services Field, plus a minimum of three years working with individuals with intellectual disabilities
  • Five years of supervisory experience
  • A valid CT driver’s license for three or more years, and the ability to obtain and maintain approval from vehicle leasing vendor
  • Must obtain and maintain CPR, First Aid, and Medication Administration Certification
  • Experience in a residential program preferred

Salary: Full Time, Exempt $37,440-$41,600 DOE


Residential Supervisor
The Residential Supervisor provides support and leadership to the residential counselors; maintaining a high level of professionalism at all times, leading by example when interacting with residents and staff. The Residential Supervisor is responsible for ensuring for the residents’ medical needs and providing direct care to residents as needed in order to meet their programmatic needs. The Residential Supervisor is responsible for ensuring that direct care staff is performing their duties in a manner that is caring and compassionate, as well as, appropriate to the individual and/or setting.

Requirements include:

  • Associates degree and three years’ experience with individuals with intellectual disabilities, or bachelor’s degree with one year experience
  • One year supervisory experience
  • DDS Medication Administration Certified
  • PMT Certified, CPR and First Aid Certifications, or must be able to obtain and maintain within six months of hire
  • A valid CT driver’s license for three or more years, and the ability to obtain and maintain approval from vehicle leasing vendor

Salary: Full Time, Exempt $37,440 - $39,520 DOE$18.00 -$19.00 per hour


Case Manager
Assist in the development and implementation of client Individual Plans (IP) by collecting and analyzing data, writing client programs, developing strategies, teaching coaches how to implement programs and help clients attain identified objectives.

Requirements include:

  • Bachelor’s degree, plus a minimum of one year working with similar populationsd, or associates degree in related field and minimum of two years working individuals with intellectual disabilities.
  • A valid CT driver’s license
  • Eligible for certification in CPR, PMT, and First Aid

Salary: Full time Exempt $30,940 - $34,580 DOE


Head Start Position

Family Worker - Bilingual
The Family Worker implements Head Start Performance Standards related to family and community partnerships while maintaining a caseload of Head Start families.  Follows program protocol for holistic delivery of services to children and families.

Requirements include:

  • A bachelor's degree in Human Service or related field and two years experience in social services; or an associates degree in Human Service or related field, commitment to further education and three year'sexperience in social services
  • Must be able to speak, read and write fluent Spanish and English
  • Ability to work flexible hours
  • Must have a valid driver’s license and access to a car
  • Knowledge of social services and community resources
  • Ability to work cooperatively, with supervisor and other staff, as a team member to build positive school-family partnerships
  • Ability to multi-task, keep accurate records and meet time line requirements
  • Ability to relate well to people and capable of inspiring confidence
  • Computer literacy

Hours: 35 hours per week, Full Year, Flexible between 7:30am and 5:30pm with some evenings

Salary: $16.00 - $18.00 per hour, DOE


Administration Position

Bi-Lingual Receptionist (part time):
The receptionist will answer, screen and forward any incoming phone calls while providing basic information when needed. Process payments for Head Start and Child Youth Services. Receive and sort daily mail/deliveries. Maintains calendar for room requests, form supplies, credit card distribution, monitoring logbook and issuing visitor badges and assists agency administrative staff in other areas as time permits.

Requirements include:

  • Bilingual in English and Spanish (ability to read, write and speak English and Spanish)
  • Experience with multi-line telephone
  • Proficient in Word and Excel
  • Professional appearance High School degree/GED

Hours: 25 hours per week, Mon-Fri, 12:30pm – 5:30pm

Salary: $12.00 per hour, DOE
